The layout
Header (employer, employee, period, pay date), earnings table (type, hours, rate, current, year-to-date), deductions table (pre-tax, taxes, after-tax; current and year-to-date), net pay, and the two taxable-wage bases. It is the layout most states' wage-statement rules are satisfied by, and the one payroll systems print. Print the page for a blank; use the generator to fill it from a calculation.
Filling it by hand
Gross from hours × rate or salary ÷ periods. Section 125 deductions first; then FICA at 6.2% and 1.45% of gross minus those; then federal withholding from Publication 15-T for the W-4 on file — the walkthrough prints every step — then state from the state's tables, then local, then after-tax deductions. Net is what remains. Year-to-date columns add the current period to the previous stub's year-to-date.

- Pay period and date, gross with hours and rate, each deduction and tax itemised, net pay, year-to-date totals, and the employer’s name; the strictest states add the address and accrued leave.
